By using technology, weᅢᄁ¬ツᆲ¬トᄁve built a mobile application that provides targeted incentive plans to users, and delivers reminders and information that help build healthy habits and foster better disease management; ultimately leading to lower cost for payers and rewards for consumers. We guide patients to learn to behave in a way that can get them better, faster; and incentivize them to adhere, engage, and manage their own health. This method connects and improves a care manager's efficiency to succeed in delivering higher touch care. We can save health plans and risk-bearing providers billions and mitigate the risk they take by putting focus on preventing avoidable costs in their highest risk populations. In helping patients to improve adherence to their care plans, we are able to drive down hospitalizations, readmissions, and complications and produce meaningful cost savings for our customers; often within 3 months in our program. We serve the most common chronic conditions with disease-specific behavior change programs, and all our programs are fully compliant with Commercial, Medicare, Medicaid, HIPAA, Beneficiary Inducement, and other important regulations.
