What Is Tri-County Care CCO?
Tri- County Care is a newly designated Care Coordination Organization Health Home- CCO/HH (one of only seven designated by New York State). Tri-County Care CCO, sponsored by the New York State Hamaspik Association or NYSHA, is proud of its rich heritage working with the I-DD community for over two decades. Our services, funded by the Office for People with Developmental Disabilities or (OPWDD), include: MSC, residential and respite care, community habilitation, family supports, day services, EMODs, ABA, Article 16, self-directed care services and vocational, as well as, job coaching services. Additionally, under the NYSHA affiliated organizations we provide New York State Department of Health (NYSDOH) supported services including: home care, personal care, health home care management, rehabilitation, weatherization, access to home, senior dining, social day care and Managed Long Term Care or MLTC services.
