Teamcare, CenterLight Healthcare’s flagship Program of All-Inclusive Care for the Elderly (PACE), is one of the oldest and largest such programs in the United States. \n\nWHO WE SERVE\nCenterLight Teamcare is a Medicare-Medicaid plan for participants aged 55 years and older, who require long-term care.
