WHO WE ARE\nCenterLight Health System is a leading non-profit provider of health and related services to older adults in need across the New York metropolitan area. \n\nOur subsidiaries include CenterLight Healthcare, an innovative managed long-term care company; Best Choice, a licensed home care services agency; Direct, an Institutional Special Needs Plan; and senior housing. Teamcare, CenterLight Healthcare’s flagship Program of All-Inclusive Care for the Elderly (PACE), is one of the oldest and largest such programs in the United States. \n\nWHO WE SERVE\nCenterLight Teamcare is a Medicare-Medicaid plan for participants aged 55 years and older, who require long-term care.
