Excelsior Integrated Medical Group (EIMG) physicians are committed to high quality care, value based medical care delivery, population management, healthcare cost reduction, by enabling its physicians to share information manage patient outcomes more efficiently and effectively. We are one of the largest multi-specialty medical groups in New York. EIMG’s 68+ providers cover over 50,000 lives in the New York Metro Area and includes 43 PCP and physician extensors, 23 specialists, 18 divisions, 22 primary care sites, and 16 specialty sites. All our PCP and established primary care sites are certified Patient Center Medical Home (PCMH) level 3, earned Diabetes Recognition by NCQA as diabetes treatment experts and received a New York State DOH grant.
