What Is Coordinated Behavioral Care?

Coordinated Behavioral Care (CBC) is comprised of 57 results-driven New York City organizations that are experienced in providing specialized case management, supportive housing and neighborhood-based clinical treatment for medical, mental health and substance abuse disorders. As a consortium of progressive, non-profit, community behavioral health providers, CBC is dedicated to realizing the unprecedented opportunities under Medicaid redesign to improve the quality of care for consumers and their families, while reducing preventable hospital admission and readmission and emergency services use by people with serious mental illness, chronic health conditions and/or substance use disorders. CBC member agencies – leaders in New York’s behavioral health service system – are committed to flexibility in service models addressing individual’s needs, including family and peer participation. CBC strives to play a significant role in achieving New York State’s projected Medicaid savings through its partnerships with managed care organizations, health systems, hospitals, ambulatory health care providers and other organizations to reduce overall Medicaid and Medicare expenditures for recipients with multiple medical, behavioral and social challenges. Our objectives are to:  Address the gap in the continuity of care for persons discharged from hospitals to outpatient services;  Shorten inpatient treatment by creating community-based alternatives;  Provide risk management training to providers to help better identify, assess and address psychiatric and medical risk, including suicide prevention;  Manage clinical and social crises that threaten community stability;  Integrate and coordinate behavioral health and medical care; and  Provide preventive health education and wellness/disease management skill training.
