Hospice of the Bluegrass is now Bluegrass Care Navigators. Having cared for thousands of families in our communities since 1978, we’ve expanded our service offerings to provide expert care long before life’s final months. While we now guide and provide care to more people, in more ways, at earlier stages of illness, yet we’re still the same provider with the same people. We have the same compassion and commitment that defined Hospice of the Bluegrass and we have a new name to reflect our growing range of services. Our current services include: • Bluegrass Extra Care - Private pay, personalized service for clients in the patient home, nursing home or assisted living facility. Services include homemaker/companionship, certified nursing assistants, sitters, skilled nursing and more. • Bluegrass Transitional Care - A program that helps patients transition to care at home after a hospitalization or from hospital to nursing facility. • Bluegrass Palliative Care - A consulting service that provides specialized medical care focused on providing relief from the symptoms and stresses of a serious illness with the goal of improving quality of life. • Bluegrass Hospice Care – A program that provides expert care during the final months of life. Includes medical, emotional and spiritual support for the patient and entire family. • Bluegrass Grief Care – A unique counseling service for support and guidance through all stages of grief including anticipatory grief and bereavement. Bluegrass Care Navigators is also proud to be a national leader in hospice and palliative education; coordinating one of six Palliative Care Leadership Centers in the United States.
