Established in 2005, PACE Organization of Rhode Island is a nonprofit health plan serving adults 55 and older who have chronic health needs and want to remain at home in the community. Today we serve over 275 participants from all over the state.
PACE is unique because it brings together a coordinated team of doctors, nurses, social workers and therapists to provide care that addresses the specific medical and social needs of older adults. The result is higher quality care and far more favorable outcomes compared to traditional care options.
As a fully integrated healthcare option regulated by the Centers for Medicare and Medicaid Services (CMS), our mission is to preserve and sustain the independence of older adults who have significant health needs and wish to remain in the community.
