We operate Summit Orthopaedic Home Care, Summit Neurologic Home Care, Summit Infusion Services, Summit Care Management, Summit Hospice, and an Assisted Living/Independent Living division branded under Summit Home Care.\n\nWe are focused on reducing hospital readmissions by using predictive analytics and real-time software to identify variables found to be contributory to hospital readmission. We're proactive in addressing changes in risk profiles in effort to keep people independent in their homes.
