At Lumina, care begins with our people — a team of specialized physicians, nurses, social workers, chaplains, physical therapists, bereavement specialists and an ever-growing family of trained community volunteers. Together, these are the people who deliver the very best quality of care and respond to the needs of our Mid-Willamette Valley neighbors. Each and every day. Since our founding, we remain an independent, non-profit 501(c)3 source for responsive community hospice care and support. We are not a subset of a large, profit-motivated hospital or hospice chain. As a charitable organization, Lumina Hospice & Palliative Care is funded — in part — by the generous support of our local community. In turn, this enables us to provide the highest in quality care for our neighbors while exceeding the levels required by Medicare and private insurance. Our services: Hospice Care - A supportive approach to end-of-life care provided at the patient’s residence. Palliative Care Case Management - An interdisciplinary medical program for patients living with a serious or chronic illness who are not yet ready for hospice Palliative Care Consults - Clinical guidance for medical providers and patients to assist with symptom management and advance care planning Transitions - A non-medical community program to help individuals live well with a serious, life-limiting illness. Caregiver Support - For those caring for a family member or friend through a chronic or terminal illness. Grief Support and Education – Counseling and groups for the entire community. End-of-life Education – Trainings and presentations tailored to specific audiences.
