The Community Rehabilitation Center is a nonprofit social services agency that offers behavioral and primary care, and community economic development services for under-served, low-income individuals with mental illness, substance abuse and HIV/AIDS who live in Duval, Nassau, Clay and St. Johns counties. More than 300 adults and families use CRC services at the center, in their adult-living facility, at home and elsewhere in the community every day.
