Founded by area churches on the principles of providing Christ-centered care in 1950, these values still hold strong and true today. We are governed by 18 Sheboygan County churches, and supported by many others. We are called by God to be of service to others, with love, compassion, and excellence, in Christ’s name.
Pine Haven Christian Communities includes 3 campuses:
Our Giddings Avenue Campus is located at 531 Giddings Avenue in Sheboygan Falls. This campus is the site where our original 12 room nursing home opened in 1950. It currently offers 115 rooms for assisted living, and 22 rooms for assisted living with memory care. Our skilled nursing home is also currently located on this campus with 50 room for long term care and rehabilitation.
Our Haven Drive Campus is located at 1280 Pine Haven Lane. This campus currently offers 40 apartments for supported independent living. An expansion is currently underway at this campus, which will house a full continuum of care. The skilled nursing will move to this campus, with two households for long term care (18 rooms each), and a separate rehabilitation household of 14 rooms, with a state-of-the-art therapy gym. The expansion will also provide 32 rooms of memory care - 24 for stage 2 and stage 3 memory care, and 8 for those who have both memory care and high physical acuity needs. An additional 8 room traditional assisted living household is also included on this campus.
Our Pine Drive campus is located at 701 Pine Drive in Oostburg, which provides 30 rooms of traditional assisted living.
