About
The Central Local Health Integration Network (LHIN) is a crown agency of the Government of Ontario. We plan, fund, deliver, coordinate and integrate health services to support over 1.9 million people living across the highly diverse communities of north Toronto, parts of Etobicoke, most of York Region, and South Simcoe County. We fund hospitals, community health centres, long-term care homes, mental health and addiction service providers, and community support services and work with patients, caregivers and families, as well as primary care providers and public health units to improve access to care, better coordinate services and improve people's experience with the health care system. We are also responsible for delivering home and community care services for over 90,000 patients each year. Home and community care services include nursing, personal support, therapy services, and specialized services such as rapid response nursing, palliative care and child and family services. We also help people explore other care options such as adult day programs, retirement homes, short and long-term care and supportive housing. The Central LHIN is committed to designing, delivering and funding services and solutions that improve access to health care and strengthen patient experiences and outcomes.
