Redwood Community Health Coalition (RCHC) comunity health centers serve as the health homes for 53% of the Medi-Cal population and the vast majority of the uninsured in RCHC's 4-county region of Napa, Marin, Sonoma and Yolo counties. The 17 member health centers serve over 232,000 patients, including 124,000 Medi-Cal patient and over 64,000 uninsured individuals. Fifty-one percent of patients reported are Latino, 33% are best served in a language other than English, 56% of patients have incomes under 100% FPL and only 6% of patients have incomes over the 200% FPL. Of the 232,000 individuals served by RCHC community health centers in 2016, about 57% were living in Sonoma County, 19% in Marin County, 13% in Napa County and 11% in Yolo County. The Redwood Community Health Network (RCHN) is a wholly controlled subsidiary of RCHC and was founded to develop shared services, creating economies of scale between RCHC and its member health centers. RCHC and RCHN provide our member health centers infrastructure, expertise and program support to: - Improve access to care through advocacy, outreach, and program expansion; - Improve health outcomes through quality improvement efforts and health information technology support
