360 Health + Community is WA’s fastest growing health and social service, delivering over 145,000 services from 29 locations across the state each year. As a registered charity, we use the profits from our private allied health and counselling services, specialist GP clinics and in-home support business to provide free and low cost health and community programs to those who need them most, in the areas of: - Mind care: including mental health interventions, care coordination and suicide prevention programs - Healthy living: group and individual programs to prevent chronic disease and encourage healthy lifestyles - Condition support: management, support and education for a variety of chronic conditions - Aboriginal health: promoting long-term health in communities all across WA - Specialist GP clinics: such as our mobile GP clinic, 360 Street Doctor and rural practices Our ultimate goal is to contribute to building a stronger and more collaborative health system: one that connects the health and social sectors, one that supports smaller community organisations and one that helps keep people out of hospital.
