Basin Coordinated Healthcare A certified home health agency that offers several health services including: Personal Care Services For eligible Medicaid recipients over 21 who have a medical need for caregivers to assist with the activities of daily living that can keep them independent in their home. Medically Fragile Waiver Program For eligible Medicaid children and adults who are at risk for/or have a developmental delay or disability and are in a medically fragile condition. Home health aides and skilled nursing are offered through this program. Early Periodic Screening, Diagnosis & Treatment (EPSDT) Program For eligible Medicaid recipients under 21.
