Hampton Roads Community Health Center (HRCHC), formerly known as Portsmouth Community Health Center, Inc., opened its doors as a federally qualified health center on May 14, 1995, in Portsmouth, Virginia, with two doctors, one family nurse practitioner and eleven clinical support and administrative staff.
In 2001 HRCHC., expanded its services to the city of Norfolk. During 2010 the two sites together served more than 9,709 patients with 31,399 medical and dental patient visits. Over 22% of the patients were diagnosed with hypertension, 15% with diabetes and over 45% received well child examinations.
Clinical services include but are not limited to the following:
- Treatment for both acute and chronic illnesses such as diabetes, stroke, hypertension, diet and nutrition
- Women’s health services
- Family planning services
- Prenatal care and postpartum visits
- Pediatric well baby, immunizations and school and sports physicals
- Preventive dental and restorative care for adults and children
- Medication Assistance Program for prescription drugs
- On site laboratory testing
- Case management services
