Welcome to Your Healthcare CIC. We are a not for profit social enterprise, proud of delivering patient-led, high quality health and social care community services for residents in Kingston upon Thames.\n\nWorking as a Community Interest Company (CIC), our status allows us to invest any surplus or profit back into front line services, where it is needed most by you, our community.\n\nAs a health and social care provider, we work in partnership to provide over 40 services to the residents of Kingston.\n\nSome of our health care community services are: District Nursing, Health Visiting, School Nursing, Rehabilitation, Rapid Response, Continence, Respiratory, a Tissue Viability & Leg Ulcer clinic, Integrated Care, Learning Disability Services, Physiotherapy, Podiatry and Speech & Language provision.\n\nOur social care community services include; Reablement & Shared Lives Scheme (formerly Adult Placement Scheme).\n\nWe currently deliver health and social care services on behalf of the Kingston Clinical Commissioning Group, other NHS organisations and local authorities.Your Healthcare CIC Address
