Capitol Care provides services to families caring for relatives with developmental disabilities at home. Our program consists of Self-Determination, In-Home and Own-Home supports. These services have been developed to assist those persons needing additional supports at home in the areas of social skills development, community integration, and life skills development. The program services are developed through a person centered model utilizing communication and coordination in planning for the services and needs with all those involved in the treatment planning process. Each care plan is developed through areas mentioned in the referral process, open lines of communication and goals of the individual we service. An interdisciplinary team approach is necessary to ensure the person obtains the best support possible.
