The Healthcare Safety Investigation Branch (HSIB) became operational on 1 April 2017 with the purpose of improving patient safety through effective investigations. Although funded through the Department of Health, and hosted by NHS Improvement, we act independently.
Our focus is on national learning and driving positive change across the NHS in England. We aim to develop recommendations that can be aimed at regulators, colleges and others at a system level. Objectivity and transparency are key principle, and we reflect this through the 'no blame'​ approach to investigations and the way cases are referred to us.
Investigations are carried out y a highly experienced team, led by Chief Investigator, Keith Conradi. Our team all have a varied range of backgrounds including health, aviation, and military and specialist expertise like human factors. They are adept at building rapport and engaging with a range of different groups - from patients and families to subject specialists.
