The Somerset Partnership NHS Foundation Trust provides a wide range of integrated community health, mental health, learning disability and social care services to people of all ages. Our aim is to provide you with the care and support you need for all the health conditions we treat. We will involve you and your family or carers, if appropriate, in all decisions about your care and ensure you have a record of what is agreed. The Trust employs 4,300 staff, and has a turnover of £148 million. We provide services from 13 community hospitals across the county, and own four hospital sites from which we provide mental health inpatient services. We see people in our community team bases, in GP surgeries and local clinics, in a range of non-NHS community settings and at home. Somerset Partnership was authorised as a Foundation Trust on 1 May 2008, building on the success of its predecessor organisation, Somerset Partnership NHS and Social Care Trust which was the first integrated health and social care partnership trust in England. On 1 August 2011, the Trust acquired Somerset Community Health, the arm’s length community health service provider arm of NHS Somerset. We are proud of our services and of the hard work and commitment of all our staff, but we know that we can always improve and develop our services.
