Medical Home Network (MHN) is a not-for-profit healthcare innovations organization dedicated to improving access and disparity issues in the safety net through collaboration, technology and an integrated, team-based model of care. In 2014, Medical Home Network founded MHN ACO, a Medicaid Accountable Care Organization comprised of 9 Federally Qualified Health Centers and 3 Health Systems in Chicago. These twelve provider member organizations operate as equal partners, committed to improving the whole-person health and quality of care for approximately 120,000 Medicaid patients in Chicago.
