NorthShore Health Centers is a non-profit organization dedicated to providing quality, affordable healthcare to the uninsured and underinsured in Porter and Lake Counties. NorthShore currently has three locations providing medical care. Patients are seen on both a scheduled and walk-in basis. After-hours services are also available through Urgent Care at two office locations, the Scottsdale and Lake Station Centers. Two dental offices, one located in Portage and the second in Lake Station provide preventative as well as corrective dental care to the communities we serve. Our facilities employ general practitioners, obstetricians, midwives, pediatricians, nurse practitioners, prenatal care coordinators, and community outreach workers. Behavioral Health Consultants work with our Primary Care Providers to ensure that emotional and behavioral issues are addressed as well as the physical. We address your entire health and wellbeing. Other Services: Digital x-ray, ultrasound, and laboratory services are also present on-site, and a fully functional pharmacy is available at the Lake Station Center and Scottsdale Center under the direction of pharmacists employed by NorthShore. Healthy Indiana Plan (HIP) and Medicaid enrollment services are available Monday through Friday on both an appointment and a walk-in basis, and Spanish-speaking employees are at all three sites to provide Spanish-speaking patients with interpretation assistance. Service is provided regardless of ability to pay. NorthShore offers a sliding-scale based on income for those with no medical insurance who qualify. Based on yearly household income, patients can pay from a minimum of $20 (0% on the sliding scale) up to $60 (100% on the sliding scale) for an office visit. This sliding-scale does not cover lab and other outside fees, however, as those prices are set by third-party providers. In order to get a sliding fee please bring the following items:
