What Is Housecall Providers?

Housecall Providers is a Portland-area nonprofit organization and one of the largest providers of primary medical care exclusively in the home in the Western United States. Since 1995, we have made over 160,000 house calls throughout the Portland metropolitan area (Multnomah, Clackamas, and Washington counties) to patients who traditionally have been the highest utilizers of Medicare dollars, the chronically ill homebound. Our doctors, nurse practitioners, and physician assistants treat nearly 2,000 patients annually in their homes (private residences, adult or residential care homes or assisted living facilities.) The Portland metropolitan area has thousands of homebound, medically complex persons who struggle to make the difficult journey to receive primary medical care. Some, because the effort is too taxing, havent received primary medical care in years. This not only means they must endure life with chronic medical issues but also affects society at large as the lack of timely care results in costly emergency room visits and hospitalization. What sets us apart from office-based practices is that we treat our patients in their homes. In addition, we excel at: strong relationships with those we serve comprehensive coordination of supplies, equipment, and referrals integration of primary, palliative and hospice care. Housecall Providers, along with 14 other sites across the country, is currently in the final year of a five-year national demonstration project called Independence at Home (IAH). The project is testing a service delivery and payment incentive model that, if made a Medicare benefit, would "reward" practices that achieve the triple aim: better care, better health, delivered at a lower cost by sharing in the savings that home-based medicine creates.
