Heart City Health Center is a non-profit, Federally Qualified Health Center (FQHC) providing affordable, high quality, medical, dental, and behavioral healthcare to Elkhart County residents. We feature an on-site pharmacy and many support services in addition to the medical and dental. We accept Medicaid, Medicare, ACA, and some private insurances. We offer a sliding fee scale based on income and family size for all patients. A Level 3 accredited Patient-Centered Medical Home, we know it's important to have a doctor who knows your specific health history and understands your individual needs throughout every stage of your life.
