Effectively managing an assigned caseload of clients\r\n â??Establishing and maintaining a positive, trusting, and ongoing relationship with clients\r\n â??Meeting with clients in their homes, physician/provider offices, and other public places in order to conduct health needs assessments and other required assessments and documentation\r\n â?? Working collaboratively with a team that includes Care Managers, Health Navigators, and Social Needs Coordinators, and the client's providers to develop and maintain a Care Plan\r\n â??Attend and participate in regularly scheduled case reviews with Care Team\r\n â??Use of mobile devices to access Electronic Health Record (EHR system)\r\n\r\nâ?¨Community Health Workersâ?¨- Job Description\r\n â??Making outbound calls and/or visiting clients at their homes, physician office, health care provider location, hospitals, etc.\r\n â??Explaining program benefits and services to potential clients\r\n â??Accurately completing enrollment documentation with clients via use of mobile Electronic Health Record system\r\n â??Use of New York State Department of Health databases to obtain and verify client information\r\n â??Assisting in client retention\r\n â??Working with an Outreach team including the Outreach Director and Community Health Workers
