Franklin County Home Health Agency, Inc. provides quality health care in your home. We have served the residents of Franklin County, Vermont, of all ages and incomes for over 50 years. Our services range from working with pregnant women, care of new borns, rehabilitation, to hospice care that provides comfort and compassion at the end of life. We are a Medicaid/Medicare certified non-profit organization that cares for over 1,700 people each year. Our funding comes from payment for services from governmental and private insurances as well as private contributions from individuals, organizations and towns to ensure our ability to provide care to anyone in need of medically necessary home health care and hospice, regardless of their ability to pay. Our Home Care and Hospice Teams work with you, your family, and doctor to create a plan of care that meets your health needs, goals and lifestyle in the comfort of your home. The Home Care Team may include: Skilled Nurses, Licensed Nursing Assistants, Physical Therapists, Occupational Therapists, and/or Speech Language Pathologists, Medical Social Workers, and Care Attendants. Your hospice team also includes spiritual advisors, our hospice medical director, staff skilled in hospice care, and volunteers to provide companionship and respite for the family. Whether you want to recover from surgery in the comfort of your home, are a woman expecting a baby, or a family member with long term care needs, we can help promote recovery and teach you and your family how to manage your illness and improve your independence. Through our Strong Families Vermont program, we pair expectant moms with skilled nurse home visitors to help provide the best life for your baby. We can help you or a family member gain physical, occupational and speech skills and keep you well by providing foot clinics and other wellness programs.
