Foundations Behavioral Health Services is a community-based behavioral health center serving Mercer and Auglaize Counties and surrounding areas. Established in 1975, we have been providing professional counseling and support services to local residents for 35 years. Originally known as Mercer County Mental Health Center, we changed our name and our focus in 2002 when we moved to our new facility just west of Celina.
