What Is Community Care of the Lower Cape Fear?

At Community Care of the Lower Cape Fear, we are changing the healthcare experience by changing the way care is delivered. We strongly believe that the best system is rooted in the communities it serves, and we know that efforts directed by doctors and focused on local patients make quality care more efficient and cost-effective. Community Care of the Lower Cape Fear ("CCLCF") was launched in 2003 as an independent 501(c)(3). Based in Wilmington, North Carolina, our geographic footprint spans Bladen, Brunswick, Columbus, New Hanover, Onslow and Pender counties. We are a regional network of nearly 550 primary and OB care practices, hospitals, health departments, departments of social services, and other agencies and organizations, including Trillium and Eastpointe MCOs and South East AHEC ("SEAHEC"). These professionals work together to provide cooperative, coordinated care through the Medical Home model. This approach matches each patient with a primary care provider who leads a health care team to address the patient’s unique health needs. CCLCF care managers provide services to high-risk patients to improve self-management of chronic health conditions. Our goal is to improve health outcomes and quality of life for our patients, while reducing care costs in the communities we serve. CCLCF is subcontractor of Community Care of North Carolina, a statewide healthcare partnership with boots on the ground in all 100 of NC's counties, and over 5,000 primary care providers participating statewide.
