Central and Eastern Sydney PHN (CESPHN) has been established to improve the efficiency and effectiveness of health services delivered to individual patients and funded by the Commonwealth. CESPHN will achieve this by working directly with general practitioners, other primary care providers, secondary care providers and hospitals to ensure improved outcomes for people as a result of: - more effective services provided for identified groups of people at risk of poor outcomes; and - better coordination of care across the local health system with people requiring assistance from multiple providers receiving the right care in the right place at the right time. CESPHN is continually working to identify, and then address local health needs, making it easier for consumers to navigate their local health care systems. This is achieved through building and strengthening partnerships with GPs, allied health professionals, nurses, local health districts, non-government organisations and our local communities. OUR AREA CESPHN's catchment spans 632 square kilometres, with a region stretching from Strathfield to Sutherland and as far east as Bondi, servicing a population of more than 1.4 million individuals.
