A mosaic of geographic and cultural diversity, and home to over 920,000 local residents, the Central West LHIN plans, integrates, funds and monitors the local health care system in the communities of Dufferin and area, Bolton-Caledon, Brampton and area, Bramalea and area, and north Etobicoke, Malton, west Woodbridge. Through the Patients First Act (2016), the Central West LHIN is also responsible for the delivery of home and community care services, and primary care planning within its geographic area. Together with a broad spectrum of partners, the Central West LHIN is committed to the development of an integrated system of health services that places patients first, responsive to their needs while delivering better value for money. À propos de nous Mosaïque de diversité géographique et culturelle et comptant plus de 920 000 résidents, le RLISS du Centre-Ouest assure la planification, lintégration, le financement et la surveillance du système de santé local dans les communautés de Dufferin et les environs, Bolton-Caledon, Brampton et les environs, Bramalea et les environs, Etobicoke Nord, Malton et Woodbridge Ouest. Aux termes de la Loi de 2016 donnant la priorité aux patients, le RLISS du Centre-Ouest est aussi chargé de la prestation des services de soins à domicile et en milieu communautaire ainsi que de la planification des soins primaires dans sa région géographique. De concert avec les fournisseurs de services de santé et les partenaires communautaires locaux, le RLISS du Centre-Ouest sest engagé à élaborer un système intégré de services de santé qui donne la priorité aux patients et répond à leurs besoins tout en optimisant les ressources.
