What Is Adirondack Health Institute?

AHI builds collaborative affiliations and partnerships to emphasize prevention, increase primary care, build more community-based options, strengthen coordination and communication, support critical safety net providers, and monitor and reward quality. Our mission is to promote, sponsor, and coordinate initiatives and programs that improve health care quality, access, and service delivery in the Adirondack region. Based in Glens Falls, NY, AHI serves a nine-county region: Clinton, Essex, Franklin, Fulton, Hamilton, Saratoga, St Lawrence, Warren, and Washington. Our programs include the Adirondack Rural Health Network (ARHN), Delivery System Reform Incentive Payment (DSRIP) Program, AHI Health Home/Care Management, Population Health Improvement Program (PHIP), and Enrollment Assistance Services and Education (EASE). The precursor organizational structure and program provider entity was founded in 1987; the transformation to AHI occurred in 2011 when three New York State Adirondack region health care entities (Adirondack Health, Hudson Headwaters Health Network, and UVM Health Network - Champlain Valley Physicians Hospital) realized the need for a regional solution to transforming the care delivery system. Glens Falls Hospital joined the partnership in 2014 and St. Lawrence Health System became the fifth member in 2015.
