Colorado Health Network, Inc (CHN) is a statewide nonprofit with two offices in Denver and offices in Fort Collins, Colorado Springs, and Grand Junction. CHN also has satellite offices in Greeley and Pueblo. These locations include the offices of Colorado AIDS Project (CAP) and Howard Dental Center (HDC). \n\nThe mission of Colorado Health Network, Inc is to equitably meet the evolving needs of people affected by HIV and other health conditions through prevention, care, and advocacy. \n\nColorado Health Network, Inc supports over 5,000 clients living with HIV and othe health conditions across Colorado access our comprehensive support services including: Medical and Oral Health Care, Case Management, Behavorial Health and Substance Abuse Counseling, Housing/Financial Assistance and Food Banks. Each support service includes the ultimate goals of working with individuals is to provide linkage to care so they can make healthy choices, increase their stability, and access healthcare.
